Overcoming Barriers
Our primary goal of case management is first and foremost focused on improving the patient’s clinical, functional, emotional, and psychosocial status.
Intensive Case Management is a partnership in our community utilizing all of our resources in our community, collaborating, seeing that our patients receive the support and help in overcoming common barriers in their lives that often prevent them from receiving needed services. This may include assistance with picking up medication, scheduling medical and mental healthcare, HIV, hep C healthcare appointments, obtaining California Identification cards, DPSS service, Social Security cards, birth certificates, completing SO applications and more.
